How/Where to Submit Records to FAA

If your most recent medical has expired or will expire within a few weeks, or if you are applying for your first medical, have any necessary medical reports with you when you see your Aviation Medical Examiner (AME) for the FAA physical examination.  The FAA will no longer review medical records or complete a review of medical records unless there is a current medical application on file.

If the AME determines that a medical certificate cannot be issued in the office, the application will be deferred to the FAA for review.  It’s best to send the supporting medical records to the FAA yourself after the AME has transmitted your physical examination report. 

Make a copy of all records for your own files, and submit all the reports together as a single packet. Office supply stores can assist you with copying large amounts of records. Your name, address, and date of birth should be on every page. If you have a PI# from previous correspondence with the FAA, use it instead.

Computer generated adhesive labels can save time if you’re sending a large file.  Delays in processing medical records often occur because the FAA doesn't receive all the required information.  If you have questions about what should be sent, call the AOPA Pilot Information Center at 800-872-2672 for assistance.

Mailing: Select a courier service that provides you with signed proof of delivery.  Use either overnight or registered mail delivery and send to:

Faxing: If your information is less than 20 pages and contains no electrocardiographic tracings, call the AOPA medical department (1-800-872-2672) for the correct FAA fax number.
