The FAA now requires a signed consent for a medical status request. The requested information and your signature allows us to conduct an inquiry on the status of your medical certification review. All information with an asterisk * is required before we can conduct an inquiry for you. The more information you provide, the easier it will be to locate your file at the FAA.
Information received by AOPA is confidential. By submitting a signed request consent, you are authorizing AOPA to act in your behalf to determine the status of your medical case review with the FAA. We will notify you, preferably by e-mail, when we receive an update on your case. If you hear something from the FAA before we contact you, please let us know so we can update our records.
The status request form allows you to provide us with the necessary information to conduct a status check on your records.
