Now submit your NASA form online
Pilots who are involved in an aviation incident can file NASA's Aviation Safety Reporting System (ASRS) form online. The new electronic submission form will give you confirmation that the document was successfully submitted. Pilots should keep this confirmation as proof that the document was filed.
If you choose to mail the NASA form, you can use AOPA's online interactive form or the original ASRS form; these forms cannot be sent electronically. Keep in mind, whichever method you use to submit the document, it must be filed within 10 days of the incident.
